Burroughs has a secure electronic health records system. Parents are able to complete the majority of the required forms online. Others will need to be downloaded and submitted. Having current and comprehensive information on your children allows us to provide the best care for them and provide a safe school environment for all. If your children has any additional health needs at school or if you have any questions about the medical forms, please contact Casie Tomlinson, school nurse.

We depend on you to submit accurate and updated health information every year. ALL PHYSICALS for the 2017-2018 school year must be completed and dated after February 1, 2017, a mandate by the Missouri State High School Activities Association.
All rising 8th grade students and all rising 9th-12th grade students not participating in a fall sport: ALL FORMS ARE DUE NO LATER THAN THE FIRST DAY OF BOOK SALES. Students will not be able to receive their schedules and thus purchase their books without a completed health form.
All rising 9th-12th grade students participating in a fall sport: ALL FORMS NEED TO BE RETURNED BY JULY 31, 2017 the first possible day for fall sports practice. Students will not be able to participate in school sports until these forms are completed.
